This is totally anonymous - no submitter ID information is  passed on to the originator of the form.

The form is to give targets of organized stalking and  electronic assault some idea of how the PUBLIC perceives  their claims.  This is difficult to find out, and so the form above has been posted for that purpose.

Thanks to all who take a minute to share their opinions.
